Evidence: Two cabinets in the hallway were not locked. Cabinet #1, the lock was broken and had two containers of Clorox cleaner, one container of comet, one container of glass cleaner. Cabinet #2, the lock was not fastened and had two Clorox cleaners, one container of Lysol, two containers of glass cleaner, and two containers of disinfecting wipes.

Evidence: During lunch on 12/05/2023, Staff #1 placed a hot cup of ramen noodles within reach of child #1 (3 years old). Staff #1 directed the child not to touch the hot noodles and began to walk away to get a bowl when child #1 began screaming and crying. Staff #1 reported that the child knocked over the cup and the contents spilled over onto the table and the child. 2. Child #1 was transported to a local hospital, admitted, and treated for second degree burns on his left calf and thigh. 3. Staff #1 and staff #3 stated that the child was transported to the hospital for burns sustained at the center.
Evidence: The record for child #4 (enrolled: 7/18/2023) had documentation of immunizations on 7/19/2023. Documentation of immunizations must be received before the child can attend the center.
