Based on staff interviews, the center failed to ensure that in areas used by children of preschool age or younger, electrical outlets shall have protective covers.
Evidence: On 04/10/2024 the preschool classes went into The Hart Gym to play prior to dismissal. At approximately 2:56 pm Staff 3 observed Child 1 (5 years old) walking to the wall. Child 1 then stuck a metal hair barrette into an uncovered electrical outlet. Staff 3 stated that they heard a pop and observed a spark from the electrical outlet. Staff 2 stated that they heard a pop. Child 1 was immediately checked for injury and taken to the school nurse. Child 1 did have some black soot on their right index finger but did not appear to be injured. There were a total of 14 children, ages 3 years-5 years, present in the gym when the electrical outlet was uncovered and the incident occurred.
