The provider was out of compliance with R381-100-10(1)(c) by failing to maintain ratios and exceeding the number of children in their single age group. In the infant/toddler classroom, there were 6 children with one caregiver. This is a repeat noncompliance as noted on 10/04/2023.
The provider was out of compliance with rule R381-100-19(12) by not ensuring that stationary play equipment is placed on protective cushioning. In the outdoor area, there was play equipment that was placed on artificial turf. This was a repeat rule noncompliance as noted on February 7, 2025.
The provider was out of compliance with rule R381-100-6(10) by not ensuring that a child had a completed admission and health assessment form before being admitted into the program. At the facility, a child did not have a completed admission and health assessment form.
The provider was out of compliance with rule R381-100-14(9) by not having the documentation for fire and disaster drills on-site for review. At the facility, there was no documentation on-site for review of the previous 12 months of fire and disaster drills. This was a repeat rule noncompliance as noted on August 9, 2024.
The provider was out of compliance with R381-100-14(13) by not submitting a critical incident report to OL within one business day, when a child was injured while in care and received medical attention. On April 28, 2025, a child in care fell and received an injury to their head that required stitches, and this was not reported to OL.
The Provider was out of compliance with R381-100-11(1)(b) by failing to ensure each caregiver provides and maintains active supervision of each child and for school-age children, a caregiver can hear the children and is close enough to intervene. During the inspection, in the Dragonfly classroom, there were 13 school age children and no caregiver.

The provider was out of compliance with this rule by not ensuring that each child being transported is in an individual safety restraint. During transportation, the children were transported in a vehicle that did not have a child restraint for every child being transported.
