The Licensee was out of compliance with rule R430-90-19(9)(a-b) by having a ladder in the use zone of a trampoline. In the outdoor play area, there was a ladder in the use zone of the trampoline. This was a repeat rule noncompliance as noted on 5/23/2024, 1/22/2024
The Licensee was out of compliance with rule R430-90-19(6) by having objects within the use zone around the trampoline. During the inspection, stationary play equipment was measured 12 inches away from the trampoline.

The provider was out of compliance with this rule by having an accessible trampoline without proper safety features. In the outdoor area, the trampoline frame and several springs did not have shock absorbing pads covering them and there was a ladder in the use zone.

The provider was out of compliance with this rule by not ensuring infants sleep in equipment designed for sleep. At the time of the inspection, the provider stated they sleep infants on a regular bed.
