The Licensee was out of compliance with R430-90-13(4) by having choking hazards accessible to children younger than 3 years of age. In the kitchen, there was a bottle cap that fit into the choke tube. This was a repeat rule noncompliance as noted on 2/8/2024 and 8/28/2024.
The Licensee was out of compliance with R430-90-13(3) by having sharp objects accessible to children. In the kitchen, there were sharp knives and adult scissors in the drawers. This was a repeat rule noncompliance as noted on 8/3/2023, 2/8/2024, and 8/28/2024.

The Licensee was out of compliance with R430-90-23(7)-(a)-(c) by not ensuring that wet or soiled diapers are disposed of properly. At the time of the inspection, there was a wet diaper on the floor in an accessible upstairs bathroom.
The Licensee was out of compliance with R430-90-19(3)(a) by having items in the three foot use zone of stationary play equipment. In the downstairs play area, there was stationary play equipment within three feet of a wall, toys, a gate, and nap cots. This was a repeat rule noncompliance as noted on 4/26/2022, 2/8/2024, and 8/28/2024.
The Licensee was out of compliance with R430-90-14(6) by not conducting required drills. At the time of the inspection, the Licensee had not completed a disaster drill in the last twelve months.
