The provider was out of compliance with R381-100-19(4)(a)-(c) by not ensuring that stationary equipment had the proper use zones. On the toddler and 2s playground, the use zone around a piece of play equipment was less than the required 6 feet. This was a repeat rule noncompliance as noted on 1/11/2023

The provider was out of compliance with this rule by having accessible safety hazards. In room 4, there was an empty plastic bag in a drawer that was large enough for a child's head to fit inside. This was a repeat rule noncompliance.
The provider was out of compliance with this rule by having children who were not actively supervised. In room 4, the caregiver did not know the number of children in their care; they stated that they had 23 children and there were 24 children in the group.
The provider was out of compliance with this rule by not having required caregiver training completed. At the time of the inspection, one caregiver had not completed preservice training offered by the department before beginning caregiving duties. This was a repeat rule noncompliance.
