The Licensee was out of compliance with R430-90-7(4)(c) by not ensuring that required training was completed. At the time of the inspection, there were two caregivers who had not completed pre-service training and were caring for children. This was a repeat rule noncompliance as noted on: 3/28/2025.
The Licensee was out of compliance with R430-90-7(2)(a)-(b) by not ensuring provider designee requirements were met. At the time of the inspection, the provider designee had not completed designee training. This was a repeat rule noncompliance as noted on: 3/28/2025.

The Licensee was out of compliance with R430-90-13(9) by not ensuring that toxic or hazardous chemicals were inaccessible to children. In the kitchen, there were bottles of household cleaners and dish soap in a cupboard under the sink.

The Licensee was out of compliance with R430-90-13(7) by having balloons accessible to children younger than five years old. In the play area, there was a balloon on the ground.
The Licensee was out of compliance with R430-90-13(4) by having choking hazards accessible to children younger than 3 years of age. In the kitchen, there were cake decorating tools that fit into the choke tube.
The Licensee was out of compliance with R430-21(9) by not keeping current animal vaccination records on-site. At the time of the inspection, the Licensee didn’t have vaccination records for four dogs and one cat who lived on site.
The Licensee was out of compliance with R430-90-23(6) by not ensuring caregivers wash their hands after each diaper change. At the time of the inspection, a caregiver changed a diaper and then sanitized their hands and changed another diaper without washing their hands.
The Licensee was out of compliance with R430-90-23(5) by not sanitizing the diapering surface after each diaper change. At the time of the inspection, the caregiver did not sanitize the diapering surface after a diaper change.
