The Licensee was out of compliance with R430-90-19(13) by not ensuring that there is no strangulation hazard on or within the use zone of any piece of stationary play equipment. In the outdoor play area, the "S" hook on the bucket swing on the playset had a gap larger than .04 of an inch.

The licensee was out of compliance with R430-90-13(4) by having choking hazards accessible to children younger than three years old. In the downstairs play area, there were several choking hazards accessible. This was a repeat rule noncompliance as noted on: 6/9/2022, 4/2/2024, and 9/24/2024.
The licensee was out of compliance with R430-90-13(3) by having sharp objects, or points that could cut or puncture skin accessible to children. In the dining area, there was a pair of pruning clippers on the table. In the outdoor play area, there was a tree limb saw lying on the ground. The licensee was out of compliance with R430-90-11(1)(a)-(d) by not ensuring that caregivers provide and maintain active supervision of each child including, the number of children in their care at any time. At the time of inspection, the caregiver at the facility did not know how many children were in care.
The licensee was out of compliance with R430-90-10(3)(a)-(c) by not ensuring that when caring for children younger than two years old, there is at least one caregiver for every three children younger than two years old and that each caregiver cares for no more than two children younger than 18 months old. At the time of inspection, there was one caregiver with 4 children under the age of 2, of which 2 of those children were under 18 months.

The licensee was out of compliance with R430-90-6(9) by not ensuring that a parent completes an admission and health assessment form for their child before the child is admitted into the child care program. At the time of inspection, there were four children in care that did not have an admission form or health assessment. This was a repeat rule noncompliance as noted on: 6/26/2023 and 4/2/2024.
The licensee was out of compliance with R430-90-20(2)(a)-(e) by not ensuring that for each vehicle used to transport children contains first aid supplies, including at least antiseptic, bandages, and tweezers. At the time of inspection, there was not a first aid kit in the vehicle used to transport children.
The licensee was out of compliance with R430-90-19(3)(a)-(c) by not ensuring that stationary play equipment with a designated play surface that is 18 inches high or higher has cushioning that covers the entire required use zone. In the outdoor play area, the cushioning did not cover the entire use zone of the play set. This was a repeat rule noncompliance as noted on: 6/26/2023 and 4/2/2024.
The licensee was out of compliance with R430-90-14(4) by not conducting fire drills at least quarterly. At the time of inspection, the last fire drill conducted was in August 2024.
The licensee was out of compliance with R430-90-13(11)(a)-(b) by having surge protectors without protective caps or safety devices while in use accessible to children younger than 5 years old. In the family room, there was a surge protector in use, that did not have protective covers in the unused outlets accessible.
The licensee was out of compliance with R430-90-13(9)(a)-(d) by having toxic or hazardous materials accessible to children. In the dining area, there was a rechargeable battery on the fire place stoop. In the entry way, there was a bottle of floor cleaning solution. In the living room, there was a container of disinfectant wipes accessible. This was a repeat rule noncompliance as noted on: 9/24/2024.
The licensee was out of compliance with R420-90-9(20)(a)-(f) by not ensuring that buildings and outdoor areas are in good repair and safe condition. In the outdoor play area, there was a broken canopy lying on the ground.
