The Licensee was out of compliance with R381-100-11(1)(b) by failing to ensure that each caregiver provided and maintained active supervision of each child. During an inspection, the licensor identified that 2 children were left unsupervised in the center's van after returning to the center from a school pick up trip.

The provider was out of compliance with R381-100-19(4)(a)-(c) by not ensuring that stationary equipment had the proper use zones. In the outdoor play area, the use zone around the play equipment was less than the required 6 feet. This was a repeat rule noncompliance as noted on 6/6/2023.
