The provider was out of compliance with R381-100-11(6)(a)-(e) by not ensuring when video cameras or mirrors are used to supervise napping children there is a camera or mirror positioned so the staff member can see and hear the child. In the infant napping room, staff could not see the napping child on the video cameras at all times.
