The provider was out of compliance with R381-100-7(6) for failing to ensure that there is a director designee with authority to act on behalf of the director in the director's absence. During an investigation it was determined that there was not a director designee at the facility.

The Licensee was out of compliance with R381-100-11(1)(e) by failing to ensure all children received active supervision. During the inspection, the licensor identified that a child was left unsupervised in their classroom, when the other students went to the playground, when their teacher did not know the number of children in their care and account for every child. This is a repeat rule non-compliance.
The Licensee was out of compliance with R381-100-12(5)(a) for failing to ensure that interactions with the children do not include any form of corporal punishment or any action that produces physical pain or discomfort. During an investigation, the investigator identified that a caregiver kicked the chair of a child causing the child to slide away from the table they were sitting at.
