Toxic/hazardous chemical requirements
The provider was out of compliance with R381-100-13(9)(b) for failing to ensure that any toxic or hazardous chemicals were inaccessible to a child. During an investigation on March 12, 2026, it was determined through interviews and a review of medical records, that the infant came in contact with a Steramine tablet, resulting in blue-tinged vomiting and medically confirmed chemical burns to the esophagus. This is a repeat noncompliance as noted on 05/05/2023 and 05/19/2023.
