Summary of Complaint Investigation The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and a complaint investigation. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. During an unannounced visit on 11/25/2024, the Licensing Consultant observed that a classroom had children present aged 20 months to 3 years old. Children under 24 months (2 years old) cannot be grouped with 3-year-olds.

See Below for Detailed Information Observation: The 12-24 month old classroom had 15 children present. The maximum grouping for this age group is 12. 1240-04-01-.22 Specifics for Child Care Centers Corrective Action: The agency divided the toddler classroom into two classrooms. Observation: The agency is required to have a system in place to ensure supervision is adequate at all times. The agency did not have enough staff present to meet ratios in 4 of the 5 classrooms present this date. Additionally, the agency was over maximum grouping in a toddler classroom. 1240-04-01-.11 Supervision Corrective Action: The agency will complete a written weekly schedule for staff. The agency will post the names of teachers and children in each classroom. Observation: The agency was out of ratio in 4 or their 5 classrooms during this visit. The 4-5 year old classroom had a ratio of 1:18 and it should be 1:16. The 3-4 year old classroom had a ratio of 1:16 and it should be 1:13. The 12-24 month classroom had a ratio of 2:15 and it should be 2:12. The 2-4 year old classroom had a ratio of 1:9 and it should be 1:8. 1240-04-.01-.22 Specifics for Child Care Centers Corrective Action: The agency called in two staff persons to work to ensure all ratios were being met. Observation: The agency is out of ratio in multiple classrooms and over the maximum grouping in the toddler room. The owner/ Director is responsible for ensuring that qualified and capable staff are present and that they are appropriately trained to ensure they know licensing rules and regulations and can remain in compliance. 1240-04-01-.06 Staff Corrective Action: The owner will provide a written schedule of staff.
