The 15-minute timer went off and the educator visually checked the children but failed to touch them to check for breathing and body temperature.

It did not include a review of the QRIS. Technical Assistance Provided: 1. Staff should be reporting to DCS and DHS immediately when there is an expressed concern regarding physical abuse. Although the agency completed their own investigation, they did not report the issue to the proper channels in a timely manner. 2. Violations were received and include: 1. 1240-04-01-.10 Duty to Report Child Abuse and Neglect 2. Correct Action Plan (CAP) completed and Put on Notice (PON) will be emailed to Director. In the Pre-K classroom one of the toliets in the bathroom is not working properly. in addition the cabinet in the Toddler 1 classroom was unlocked and cleaning products were observed. The items were not accessible to the children, but they must be locked when not in use.

The agency was cited for ratio violation during the quarter. Points were not earned due to the critical violation. The violation has been corrected. However in the Toddler Two classroom during lunch there were two table of childen. There were two children seated at a round table. One educator was seated with childen at another table and the other educator was using the ipad at the counter. The educators could not see one of the children's faces at the round table because his back was to educator who was seated at the other table. The child fell asleep at the table; and the eductor's were unaware as they could not see his face. LC brought this to the attention of the educators. One educator moved the child to another place at the table where they were able to see his face. The child woke up and started to eat his lunch with appropriate supervision.
During lunch in the Toddler Two classroom an educator told a child if you do not sit down you will not get any food. Later, the educator told a child if you take your bib off your will not get any food.
