During a complaint investigation on June 29, 2026 the agency was cited for Duty to Report Child Abuse and Neglect. An incident was reported to the director on April 16. 2026 and it was not reported to the Department of Children's Services as required.

Summary of Complaint Investigation, Follow-Up, and Probation Monitoring Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations, complaint, follow-up, and probation. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. A child was left unsupervised on one of the playgrounds for approximately one minute. The adults responsible for supervision left the child on the playground without adult supervision. Three children were not signed into a classroom at the correct time. Two children were brought inside for diaper changes and one child was left unsupervised on the playground and brought into the classroom at a later time. Three of the children's time in the classroom were falsified on the tracking log. A child was left unsupervised on one of the playgrounds for approximately one minute. The adults responsible for supervision left the child on the playground without adult supervision. An educator had an infant who was five months old on the playground with children ages two through three years. There is no age grouping for five months through three years. A child was left unsupervised on one of the playgrounds for approximately one minute. Management did not fill out an incident report, did not inform the parent when the parent picked up the child, and did not make a report to TDHS and TDCS.
