It had not been cleaned since 5/21/18. 1240-04-03-.12 PHYSICAL FACILITIES Corrective Action: A plan of corrective action was completed on site. Agency staff cleaned the bathrooms during the PE's visit. The violation was corrected on site. Observation: The toilet/commode seat in the girl's bathroom was missing one plastic screw and was loose. 1240-04-03-.08 EQUIPMENT FOR CHILDREN. Corrective Action: A plan of corrective action was completed during the visit. The staff placed a plastic zip tie in the place of the missing screw until the toilet seat could be replaced. On 5/24/18, staff sent a picture for documented proof the toilet seat had been replaced.
