Summary of Complaint Investigation/Safety Plan Monitoring Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations, to investigate complaint #PCS1009136 and to monitor the safety plan. On 08/01/24 an incident occurred at the agency which should have been reported to the Department of Children's Services. The agency did not report the incident to the Department of Children's Services as required. On 08/01/24 an employee who was the subject of a safety plan was alone with children at the agency which was in violation of the safety plan in place at that time. On an unannounced complaint investigation visit on 08/06/24, Licensing Consultant Amy Richardson discovered that an employee was not following their safety plan which resulted in a child being hurt by the educator. It is the management's responsibility to ensure the agency is following the requirements of a safety plan to ensure the safety of children. On 08/01/24 an incident occurred at the agency with a six (6) year old child which should have been documented in the child's record and a copy of the report given to the parent. The incident was not documented in the child's record or given to the child's parent as required. On 08/01/24 the agency attempted to verify allegations of abuse by investigating an allegation themselves instead of reporting the allegation to the Department of Children's Services.
