It was discovered during an SIU investigation an educator witnessed possible child abuse. This was reported to the administrator/administration and was not reported to the Child Abuse Hotline. During SIU investigation it was discovered that an educator reported to administrator/administration that she may had witness a educator "backhand" a child. Administrator/administration did not inform the parent with written documetation that the child may had been involved in an incident. It was discovered during SIU investigation that it was reported that a child was involved in a incident that was serious and should have been reported to the TDHS or TDCS via provider portal
