Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and a complaint investigation. During a complaint investigation it was confirmed that a child was on the playground eating without an educator directly supervising; licensing rules state that an adult must be within director sight and sound the child while they are eating. During a complaint investigation it was confirmed that the maximum capacity of children for the age grouping were on the playground at one time, resulting in potential safety hazards and risks. During a complaint investigation it was determined that there were 50-52 children on the playground at one time, resulting in supervision and safety issues. During staff interviews conducted for a complaint investigation it was determined that adult:child ratios were exceeded causing classrooms to be out of ratio. One educator had too many children causing her to go over her ratio for one teacher. On the playground teachers were going in and out of the building causing classrooms to not meet adult:child ratio requirements. A serious incident was brought to the attention of the site Director; Director did not report the serious incident to DHS.

See Below for Detailed Information 1240-04-03-05 Ownership, Organization, and Administration Observation: There was no proof/documentation of one child’s immunization record or an exception as required. Corrective Action: Upon the child’s enrollment, checklist items will be required. After two weeks, the child will not be able to return. 1204-04-03-07 Staff Observation: There was no proof/documentation that a staff member received trainings for the following: shaken baby syndrome/abuse head trauma and a pre-service training recognized by the Department as required. Corrective Action: The agency will create a checklist to ensure all trainings have been completed when new hires come to the agency on the first day. Observation: The agency received nine immunization deficiencies February 27, 2018. It is required that all children enrolled at the agency have all current immunizations. 1240-04-03-05 Ownership, Organization, and Administration Corrected Plan: Parents were given a deadline to have current immunizations for the children.
