During a complaint investigation it was discovered that an educator used a form of inappropriate discipline with a child/children. During a complaint investigation it was discovered that possible child abuse and neglect had occurred. The director was made aware of this and did not report to the proper channels for this to be investigated. During this time the safety plan was not being followed in compliance due to the following being observed: -There were several staff members that did not complete training with CCR&R -The agency received violation TCA 71-3-508 due to noncompliance of the safety plan. Management did not report a serious incident to TDHS or the child abuse hotline.
