Summary of Monitoring Visit The visit conducted today included investigating a complaint/incident that occured on 05/10/2023 including a student being left on the playground. LC Holden interviewed the site director as well as reviewed surviellance footage to confirm the incident. Technical Assistance Provided: 1. Ensure that staff are using name to head count to keep up with the number of students they have and that they do it before and after every transition to and from any room/area. Summary of Monitoring Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations as well as reviewing the safety plan. LC Holden also served the site director the civil penalty for the previous violations. LC Holden will assist the Agency with any requests or assistance needed to assure completion of all tasks from the safety plan. No other concerns were observed or reported during the visit. Technical Assistance Provided: - Ensure that staff continue to perform a name to face head count for students before/after transitioning to and from areas. - Ensure that staff are reporting any incidents or concerns they have related to the students immediately to the site director.
Summary of Monitoring Visit The visit conducted today was a follow up visit from the previous week's investigation visit and also included serving of the safety plan as well as the notice of probation to the agency. The LC reviewed the Safety Plan as well as the Probation notice, and, PON letter with the director to ensure they understood what was being required and if they had any questions or needed any assistance. Technical Assistance Provided: 1. Ensure staff are performing name to face counts of students when transitioning to and from other areas. 2. Ensure that all staff are reporting incidents/accidents immediately upon them occuring to all appropriate parties (DHS, parents, etc.) No Violations were received
Summary of Monitoring Visit The visit conducted today was a follow up from the previous safety plan monitoring visit. During the previous visit, the Safety Plan was lifted and a letter of doumentation was served to the agency. Today's visit was a follow up to serve the agency their civil penalty stemming from the violations cited on February 9th. No concerns were observed or reported during the visit. All the training requirements have been met by agency staff as well as other requested items per the safety plan. Technical Assistance Provided: - Ensure that staff are constantly aware of the number of students they have at all times.

The LC reviewed the Safety Plan with the director to ensure they understood what was being required and if they had any questions or needed any assistance. Technical Assistance Provided: 1.
