Technical Assistance Provided: 1.LC discussed with director that educator on safety plan will normally always be in Fannie Frogs. Today there were many staff that were out and other staff not named as a 2nd educator were in class with Educator on Safety plan during other educators break time. 2. During an unannounced visit to investigate a complaint, after LC watched video and spoke to director it was determined that a child was left unattended outside on the walkway that leads to the classroom for three and a half minutes until the educator opened the door and brought him inside the class. The class was coming in from the playground. During an unannounced visit to investigate a complaint, after watching video of the incident and reviewing the name to face log from the classroom, it was determined that educators did not conduct a name to face roll call when entering the classroom. The class was coming from the playground.

Educator didn't conduct a name to face roll call when entering the building from the playground. A 2.5 year old female was left unattended on the playground.

(c) If any child is left unattended at any time, the child care agency is subject to summary suspension, denial or revocation of the license. This includes, but is not limited to a child: Agency self reported that a 1 year old was left outside alone for approximately 25 minutes Observation: Agency self reported that a 1 year old was left outside for approximately 25 minutes alone. . 1240-04-01-.11 SUPERVISION Corrective Action: Director terminated both educators the day of the incident. A new check sheet was implemented for tracking where the child went and the time they left and returned.

Observation: on video from October 2, 2018, observed by Helen Gamble, DHS, and Jennifer Head, DCS, on October 8, 2018, a caregiver at the agency pulled a chair out from under a six year old that was misbehaving and then repeatedly shoved him into the floor several times when he attempted to fight her back. 1240-04-01-.06 STAFF. (Observation: it was determined during an SIU investigation conducted on October 8, 2018, after viewing video footage of an incident that occurred on October 2, 2018, that a caregiver in the school age classroom initiated a physical altercation with a six year old child. She repeatedly shoved him into the floor.)
