See Below for Detailed Information Observation: There was one child age three (3) years old was left inside the restroom by staff without an adult knowledge. 1240-04-01-08 Supervision Corrective Action Plan: Agency plans to conduct a roll call during transitions using face to name form to verify that all of the children are accounted for. Observation: The agency did not have proof/documentation of parent receiving an incident report/form and in child’s file when incident occurred as required. 1240-04-01-09 Incident Reporting Corrective Action Plan: All staff will complete accident/incident reports same day incident occurs, provide report in child's file, and notify parents immediately.
