Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and Complaint Investigation Visit. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. The director failed to make a required report to DHS/DCS after she was notified that the incident occured. After seeing the camera footage, it was determined that an educator observed suspected child abuse and did not report the concerns to the correct agencies as required.

See Below for Detailed Information LC observed a child's epi pen not in the lock box. It was in the child's bag. The caregiver also wasn't able to reach the lockbox as it was placed on a high shelf. The epi pen wasn't in the original box with the child's information on it. The epi pen had the child's name on it, but no other information. There wasn't any paperwork with the medication. 5. Medication requiring emergency administration, as prescribed by a licensed medical professional, e.g. an “Epi-Pen” or asthma inhaler, may be kept in an unlocked container that is inaccessible to children. LC observed 8 two year old's in a classroom with one teacher. (1) Required Adult:Child Ratios. (a) Adult:child ratios in this paragraph shall be maintained by the child care agency while the children are on the premises of the child care agency, including outdoors or on the playground. Age of Children Adult:Child Ratio Maximum Group Size Six (6) weeks – Fifteen (15) months 1:4 8 Twelve (12) – Thirty (30) months 1:6 12 Twenty-four (24) – Thirty-five (35) months 1:7 14 Three (3) years 1:9 18 Four (4) years 1:13 20 Five (5) years 1:16 20 School-age (Kindergarten and above) 1:20 No Max 2) Meal and Snack Time Supervision. (a) During meal and snack time, staff shall maintain direct supervision of children between six (6) weeks and five (5) years of age and maintain supervision of children between six (6) and nine (9) years of age. (b) During meal and snack times, educators that are providing supervision are prohibited from performing other activities and classroom duties unrelated to food service. (c) Child care agencies shall develop and follow a written mealtime supervision plan that addresses: 1. Room arrangement that will allow staff to directly supervise each child at all times; LC observed child sitting at the table with his back to the caregiver during mealtime.
