During a complaint investigation, while LC Kelley was obtaining ratios for the classrooms, a medication in the PreK One class had expired in March 2026. During a complaint investigation, LC Kelley was made aware that incident reports had not been filed out.

During a complaint investigation it was confrimed through video that a verbal and physical altercation took place in the building. Children were not involved but could hear the altercation, there were parents that witnessed the incident During complaint investigation it was discovered that an educator was using foul language toward the children and expressing her feeling about the children in front of them.

Summary of complaint investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. During a complaint investigation. IT was determined that a child was out of direct sight and sound of the educator. During investigation of a complaint it was confirmed that an incident had taken place and was not reported to DHS. During complaint investigation it was discovered that there had been multiple occassions where infants from ages 10mths-12mths had been left in the closet for brief periods of times. During complaint investigation it was discovered that the director had knowledge of an incident that took place on multiple occassions that may have put infants in a harmful situations. Director did not report this to DCS or DHS. During a complaint investigation it was discorved that several staff member and director had knowledge of an incident that occurred. This should have been reported as possible child neglect.
