Summary of Follow-Up and Delivery of Safety Plan Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations, a follow-up of the visit from 8/1/2023, and a delivery of the Safety Plan. Technical Assistance Provided: LC discussed the Safety Plan requirements and the outstanding Corrective Action Plans. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans.

Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and a complaint investigation. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Upon interviewing the complainant and agency staff, the Licensing Consultant determined that on 4/14/2025, the agency did not inform the family that their child would be in the same classroom with the child that had bitten him on 4/10/2025. The agency had previously assured the family they would not be in the same classroom and that the parents would be contacted if it happened.

See Below for Detailed Information Observation: CCPE observed film footage verifying a caregiver in pre-k room was dragging a child across the room by her arm and being very aggressive with the child who was 5 years old. Corrective Action: Caregiver was terminated immediately. Staff were given handouts on elbow pulling and safe touches. Staff will receive training in conscious discipline and stress management.
