Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint visit. The agency self-reported that on 6/5/24 at approximately 3:04 PM, a child was left alone in the classroom for approximately 3-5 minutes. Roll Call was not performed as required by licensing rules. The agency self- reported that on 6/5/24 a 3 yr. old child was left alone in the classroom for approximately 3- 5 minutes. Agency self-reported that on 6/5/24 at approximately 3:04 PM, a child was left alone in the classroom for approximately 3-5 minutes. The ratio at the time was 1:6. Management should ensure that staff are aware of correct Roll Call procedures.

See Below for Detailed Information Observation: While investigating complaint #27520, it was determined that a staff member moved her remaining children to another classroom at the end of the day, but she left one child, age 3 years 11 months, alone, unsupervised, sleeping on a couch in her classroom for approximately 11 minutes. 1240-04-01-.06 STAFF Corrective Action: Owners, directors, and primary educators shall be responsible for selecting qualified and capable staff, and the supervision, training, and evaluation and performance management of the staff. Observation: While investigating complaint #27520, it was determined that a child, age 3 years 11 months, had been left alone, unsupervised, sleeping on a couch for approximately 11 minutes. 1240-04-10-.11 SUPERVISION Corrective Action: Agency staff will use the sign in sheet and use name to face when moving children from one location to another, so that no children will be left alone. Licensing Consultant discussed with each educator, using the sign in sheet and name to face each time children move from one location to another. Licensing Consultant made a referral to CCR&R to provide training for all agency staff regarding how to safely move children from one location to another. Observation: While investigating complaint #27520, it was determined that the agency failed to report a serious incident. 1240-04-01-.09 INCIDENT REPORTING Corrective Action: To ensure the health and safety of all children enrolled, the child care agency shall notify the Department of all serious incidents the same day of the incident.
