Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations, and complaint investigation. If any violations were received, details can be observed in the compliance details section of the visit, along with corresponding corrective action plans. A child was present and enrolled at the agency who is 1 year old. The agency license is for ages 2y-5y. The agency has a chain of command posted, but the second person posted is one of the owners, and the director is the director of two agencies LC was seated in the lobby and did not observe or hear name to face roll call as the children entered the agency.

See Below for Detailed Information Observation: Developmentally inappropriate discipline techniques were used by placing a three ( 3) year old child in timeout for up to 5 minutes outside the classroom in the doorway of the office. As required by licensing, behavior management shall be developmentally age appropriate, reasonable, never include seclusion and be based on the age of the child to not exceed one (1) minute per age. Violation: 1240-04-01-.15 Program, Language and Literacy Development Corrective Action: Positive Behavior Management and Guidance Training will be conducted with all staff. Observation: A child was placed in timeout located in an area of unlicensed space for children in the facility. Violation: 1240-04-01-.22 Specific Requirements for Child Care Centers Corrective Action: Children will no longer be removed from a classroom and placed in any area of unlicensed space in the facility.

See Below for Detailed Information Observation: One child in the 2 yr. old classroom did not have a sheet covering his mat as required by DHS. 1240-04-03-.08 Equipment Corrective Action: The director provided extra sheets to the agency to have available for all children when needed. Observation: One child in the 2 ½-3 yr. old classroom did not have a sheet covering his mat during naptime. 1240-04-01-.14 Equipment Corrective Action: The agency keeps extra sheets available for children on site and has made all staff aware if any are needed for children. Observation: Mats were not labeled with children’s names in the 2 yr. old classroom as required by Licensing all children shall be assigned his own nap equipment. 1240-04-03-.08 Equipment Corrective Action: Caregiver labeled all mats with children’s names while Program Evaluator was on site at today's visit.
