See Below for Detailed Information Due to the public health concerns with the COVID-19 virus, and pursuant to the authority in Executive Order No. 50, Child Care Licensing staff are conducting desk audits and virtual visits when appropriate to ensure healthy and safe practices of providers during these unique circumstances. All information will be documented timely. Observation: While conducting an unannounced telephonic call on 12/14/20 and an unannounced virtual visit on 12/15/20, it was discovered that a child, approximately 23 months of age, had been left unsupervised on the playground for approximately one (1) to two (2) minutes on an unknown date, sometime in the last few weeks, 1240-04-01-.11 Supervision Due to the public health concerns with the COVID-19 virus, and pursuant to the authority in Executive Order No. 50, Child Care Licensing staff are conducting desk audits and virtual visits when appropriate to ensure healthy and safe practices of providers during these unique circumstances. All information will be documented timely. Observation: While conducting an unannounced telephone call on 12/14/20 and an unannounced virtual visit on 12/15/20, to follow up on complaint #270991 and #270993, it was discovered that the agency did not complete an incident report, did not notify the parents, and did not notify Child Care Program Evaluator (CCPE) regarding a 23 month old child being left unsupervised on the playground for approximately one (1) minute to two (2) minutes on an unknown date sometime in the last few weeks. 1240-04-01-.09 Incident Reporting

See Below for Detailed Information Observation: On 7/19/2019, while Child Care Program Evaluator (CCPE) and Child Care Licensing Supervisor (CCLS) were following up on complaint #251063, an educator, stated that she observed a child’s sheet was soiled, when he got up from his nap at 2:30 PM, but she did not give him clean clothes until after he finished his snack, which was eaten from 2:45 PM until 3:00 PM. 1240-04-01-.12 Health and Safety Corrective Action: The staff was terminated by the agency. Observation: During an unannounced visit to follow up on complaint #251063 on 7/19/2019, Child Care Program Evaluator (CCPE) and Child Care Licensing Supervisor (CCLS) observed two educators removing plates of food from a cart, carrying the plates of food to where the three (3) to four (4) year old children were seated at tables, and placing the plates of food in front of the children, while neither educator was providing direct supervision to the children. 1240-04-01-.11 Supervision Corrective Action: CCR&R training was provided at agency's request-documentation in case file. One (1) staff was terminated due to additional issues.
