Summary of Complaint Visit and QRIS Monitoring Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations, to investigate a complaint and scoring of the QRIS. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. An allergy list was not posted in the preschool classroom (Little Blossoms). On April 3, 2024, there was only one signature (the bus driver) indicating a walk through was conducted after the morning and afternoon bus ride from Chimney Rock Elementary. There were several classrooms that had keep out of reach items in children's reach that were not locked away. The infant classroom had a jar of petroleum jelly on the changing table, the one's classroom had a tube of diaper cream, the two's classroom had a tube of antibiotic ointment and the young preschool classroom had a bottle of disinfectant spray on the sink. During the unannounced complaint and QRIS visit, upon arrival, children were allowed to go to their classrooms alone without supervision. The Little Blossoms classroom (2 1/2-3 years old) did not have a mealtime supervision plan posted in the classroom. A child from the Sr. Scholars was allowed to go to the restroom alone in the three's classroom. There were three (3)children's files that did not have immunization records. One child's file did not have the parents' contract signed, the child's health history form was not complete and the influenza form was not signed. There are no extra materials stored in the Little Blossoms classroom for easy use by the educator. Children's work was not displayed in the preschool (Little Blossoms) classroom. After reviewing files of staff that drives for the agency, neither of the two drivers have had an annual physical. Children's allergies were not posted in the Little Blossoms classroom. During the unannounced complaint and QRIS visit, upon arrival, children were allowed to go to their classrooms alone without supervision. Their ages ranged from two to ten (2-10) years old A school age child took an infant (11 months old) to her classroom alone. A child from the Sr. Scholars was allowed to go to the restroom alone in the three's classroom. After reviewing the bus logs, on April 3, 2025, there was only one signature (the bus driver) indicating a walk through was conducted after the morning and afternoon bus ride from Chimney Rock Elementary. In addition, it could not be confirmed if one of the drivers (Benjamin South) had the required qualifications, if he's been drug screened and had an annual medical exam due to his file not being accessible on site. A total of nine (9) infant's bottles were not labeled with both the child's name and date on them. Also in the one's classroom, there were four (4) sippy cups that were not labeled.

The two year old educator was left supervising a total of 17 children as they napped. Ten children were 2-2 1/2 years old and 7 children were 1 years old. The required ratio for one year olds is 1:7 and the ratio for two's is 1:8 or 2:16. In addition, the two's educator could not see all children napping in both rooms from where she was sitting.

The one year old educator went to lunch at the same time the second educator in the two year old classroom took her lunch break. The two year old educator was left supervising a total of 17 children as they napped. Ten children were 2-2 1/2 years old and 7 children were 1 years old. The required ratio for one year olds is 1:6 or 2:12 and the ratio for two's is 1:8 or 2:16. Ratios for children six (6) weeks through thirty (30) months shall be maintained.
