On September 22, 2025, during an unannounced complaint investigation, the Licensing Consultant discovered an individual present at the child care agency without having a criminal background clearance to work.

During the unannounced complaint investigation, safety plan delivery, and follow-up visit on July 14, 2025, the supervising educators were not present in the Educated Eagles classroom and could neither hear nor see the 15 children (ages 4 to 7 years old) when the Licensing Consultant and Field Supervisor arrived at the child care agency at 2:58 PM. On July 14, 2025 at 2:58 PM, the Licensing Consultant and Field Supervisor observed two educators supervising 15 children ages 4 to 7 years old. They entered the building at 3:03 PM. According to the ratio chart, the maximum group size for a classroom with children aged 2 1/2 to 12 years is 10 children per supervising educator.

Since the volunteer present during the observation was not included in the ratio, it indicates that no educator was present. During the unannounced complaint investigation visit on July 7, 2025, the Licensing Consultant noticed that 1 of the toilets in the boys' restroom was not safe, clean, or in good repair. During the unannounced complaint investigation visit on July 7, 2025, the Licensing Consultant did not witness a qualified adult/educator in the 1-2-year-old classroom. However, the Licensing Consultant witnessed 1 volunteer alone with 7 children in the 1–2-year-old classroom. During the unannounced complaint investigation visit on July 7, 2025, the Licensing Consultant noticed that the 1-2-year-old classroom was not following the correct adult: child ratios. In the toddler classroom, there was 1 volunteer to 7 toddlers. According to the regulations, the ratio for children aged 1-2 years should be 1:6 (1 educator to 6 children) and the educator must be qualified. The volunteer was not a qualified educator and should not be left alone with children or counted in the ratio. During the unannounced complaint investigation visit on July 7, 2025, the Licensing Consultant observed 1 volunteer with 7 children in the 1–2-year-old classroom. As a result, this put the classroom out of ratio because the volunteer could not be counted in the ratio.
