Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint investigation. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. On 10/15/2024, during the complaint investigation, the Licensing Consultant did not observe documentation that the maintenance of the vehicle used to transport children had been performed by a certified mechanic on-site. On 10/15/2024, during the complaint investigation, the Licensing Consultant did not observe documentation of daily inspections on-site and necessary repairs or other appropriate action taken before transporting children. On 10/15/2024, during the complaint investigation, the Licensing Consultant observed a new driver a physical walkthrough of a small white bus upon arrival. The department has not approved the driver or the small white bus to transport children at the child care agency.

During the complaint investigation on 9/26/2024, the Licensing Consultant discovered that an educator was excluded by criminal background from working with children. The Licensing Consultant observed the educator in the preschool classroom with another educator with 10 children ages 3 to 4 years old. The Director/Owner notified the educator to leave the premises immediately.

Ratio Requirements No Violations were received Violations were received and include: 1. The infant room was out of ratio on 3/23 (ratio was 1:6; requirement is 1:5) 2. The preschool room was left unattended on 3/23 3. Two infants were drinking from bottles in their cribs on 3/23. 4. The preschool room was left unattended during complaint investigation on 3/24. Correct Action Plans (CAP) will be completed. Two infants were observed to be drinking from bottles in their cribs on 3/23/23; infants shall always be held or placed in a high chair when taking a bottle. The infant room had a ratio of 1:6 on 3/23/23; the required ratio is 1:5. The preschool class (ages 2 - 4 years) was observed to be unattended during the complaint investigation on 3/24. The preschool room (ages 2 - 4 years) was left unattended on 3/23/23.
