CI 1.7: Building and physical premises safety

Items labeled "keep out of the reach of children" were not locked when not in use with the exception of hand soap. The playground surfacing did not meet the minimum required depth of 6". There were small slides that were on mostly grass on one playground. The other playground had pea gravel.

Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations. Technical Assistance Provided: 1. LC discussed proper documentation in the portal as it relates to self-reporting incidents and complaints. LC discussed uploading pertinent documentation as it relates to the complaint and re-emphasized the protocol when reporting incidents and complaints. 2. LC discussed written documentation as it relates to specific requests from parents (ex: who the child may be released to, what the child can/cannot eat or play with etc.). The director was receptive of TA. Violations were received and include: 1. Reports of Incidents, Illnesses, Accidents, Injuries & Fatalities 2. Health & Safety Correct Action Plan (CAP) completed. 1. A serious injury report was not completed and the procedures for reporting a serious injury of a child was not conducted. 2. The necessary steps for ensuring the safety of a child/children was not taken and/or documented.
