See Below for Detailed Information Observation: Six (6) children are present without proof of immunization, on file, as required. 1240-04-01-.08 RECORD KEEPING. Corrective Action: Parents and relatives were called to remove the children from the premise. Parents and relatives were notified children cannot return until certificates of immunization have been submitted to provider. Observation: One (1) child was present in care over twelve 12 hours in a twenty-four (24) hour period from 5:45am to 6:00pm 1240-04-01-.05 OWNERSHIP, ORGANIZATION, AND ADMINISTRATION. Corrective Action: Parents and relatives were notified, that children can only be in care for 12 hours in a 24 hours period unless special approval has been received from the Department. Observation: On 9-7-18, Logs are in violation with eleven (11) children present from 5:30am-6:05am, with one educator. 1240-04-01-.03 REQUIREMENTS FOR LICENSURE AS A CHILD CARE AGENCY. Corrective Action: Parents and relatives were called to discuss arrival and enrollment changes needed. Provider will submit the revised roster of enrolled children with their arrival times listed to ensure the agency is not over the licensed capacity.
