Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint investigation. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. Licensing Consultant (LC) conducted an unannounced visit on 07/24/25 and discovered that the infant /toddler educator did not have a record of diaper changes on the date of the reported incident (July 17, 2025). Licensing Consultant (LC) coducted an unannounced visit on 07/24/25 to investigate a complaint and discovered that there was no record of the child in the complaint's diaper being changed throughout the day. The educator failed to document diaper changes. Therefore, the complaint of the child's diaper not being changed could not be deemed invalid.

The Licensing Consultant (LC) conducted an unnanounced on 9/23/2024 due to a complaint received from an ex-employee of a child being hit by an educator. The agency failed to report the incident to the Licensing Consultant and Department of Children Services.
