Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and a complaint. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. An incident report was not done when staff noticed marks on the toddler child. Pictures were taken by the owner and sent to the custodial parent. There were two children in the infant/toddler class that did not have documentation of their feeding, toileting and sleep patterns.

Rule: 1240-04-01-.09 INCIDENT REPORTING Plan of Corrective Action: The agency has made immediate changes to assure incident reports will be made at the time of all incidents and parents will be notified immediately. Observation: A child was left in a bouncy unattended and extracted himself, falling to the floor. Rule: 1240-04-.15 PROGRAM LANGUAGE AND LITERACY DEVELOPMENT Plan of Corrective Action: The agency will assure that staff is attentive to children at all times. No child will be left in a cot, crib, or bed at any length of time while awake.
