Summary of Complaint Investigation/Safety Plan Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and Complaint Investigation/Safety Plan. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. The ratio required for infants (6 weeks - 12 months) should be 1:4. The ratio during the visit was 1:5. A video recording revealed that a two year old child was in time out for longer than 5 minutes.

Summary of Complaint Investigation Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and Complaint Investigation. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. A serious incident was not reported on the same day that it occured as required by the child care licensing rules and regulations. It was reported on the following day. A child was forcefully placed on a mat several times. The child's head was also pushed onto the mat more than once.
