educator did wash children's hand nor her own following a diaper change or in between diaper changes.-Recommend relocating the diaper-changing table to the same wall as the restroom for easier access. Ensure only the child assinged to a cot is having access/getting on that cot- during diapering multiple children got on that cot in that space but then a child was put there to rest.
