A joined two and a half to three room and two year old room as well as the three year old room. 1240-04-01-.16 PHYSICAL FACILITIES. Corrective Action: Director noted he has placed a tarp on the ceiling. Some tiles have been repaired. Director/owner has obtained a estimate for repairs and will call to schedule roof repair.

See Below for Detailed Information Observation: Educator's back was turned to the 3 children eating at the table. During this time 2 more children entered the dining room and started eating. 1240-04-01-11-Supervision 1240-04-01-13- Food Corrective Action: Educator will change viewing stance while supervising hand washing before mealtime to ensure all children remain in direct sight of supervising staff. Meal Preparation staff will notify classroom staff when food is on distributed on the dining room tables.

Observation: One (1) new employee file lacked required documentation of physical and three (3) references. 1240-04-03-.05 Ownership Organization and Administration Corrective action: Employee sent home and will not be scheduled to work until required documents are turned in to management. Required documents verified on file 4/24/2018, violation corrected. Observation: Wet ceiling tiles in café due to leak in roof. 1240-04-03-.12 Physical Facilities Corrective action: documentation provided for repairs made 4/8/2018, management has contact roofer to return and make additional repairs. 5/4/18 PE received verification from agency management that additional roof repairs were made to fix leak.
