Observation: An educator diapered a child but did not wash her hands or the child's hands afterwards. Further, the educator touched equipment and held another child's hands, spreading germs. Part of the proper diapering process is for the infant and the educator to wash hands after diaper changes, for health and safety reasons. 1240-04-01-.12 HEALTH & SAFETY Corrective Action: Program Evaluator provided technical assistance and a plan of corrective action was developed. The director will review hand-washing and diapering procedures with all staff/educators and will ensure that they comply with procedures appropriately.

Observation: Children were in a room at naptime with no lights on, completely in the dark. There needs to be some light in rooms at all times to properly supervise children. Further, in an emergency situation anyone entering the room would not have time for their eyes to adjust to pitch darkness and this could lead to a child being left behind in an emergency evacuation situation. 1240-04-01-.16 PHYSICAL FACILITIES Corrective Action: Technical assistance was again given to the agency regarding this violation, but this has been a repeated problem at visits. Today a plan of corrective action was developed to fix the problem permanently. Director states that their lights all have dimmer switches and they will use them from now on instead of turning off all lighting.

Observation: The agency has not conducted an evacuation drill since it opened, in spite of repeated reminders from Program Evaluator. Evacuation drills are supposed to be done each month, to ensure that children can get out of the building in an emergency situation. These children were placed at risk due to this on-going lack of practice drills since the agency opened in August 2018. 1240-04-01-.12 HEALTH AND SAFETY Corrective Action: Technical assistance was given to the director. A written Plan of Corrective Action was developed. The agency did a practice drill this date and a true evacuation drill this date. Documentation was posted at the agency. Director has ensured Program Evaluator that monthly drills will be done from now on to meet child care regulations.
