Informing parents of breach in confidentiality. Summary of Safety Plan Check and Probation Notice Served The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations, safety plan check, and Probation notice served. Technical Assistance Provided: 1. Food Amounts 2. Sensory Bottles for children under 3 years 3. Name to Face roll call inside of the classroom. Children under three years old had access to materials that were not considered age appropriate for their ages and abilities.

See Below for Detailed Information Observation: There was a mealtime supervision lapse during the 8-1-18 assessment when educators were doing non-food service related tasks instead of fully focusing on children while they ate lunch. Due to the possibility of choking, children need to be closely supervised while they eat in a child care agency. 1240-04-01-.11 SUPERVISION Corrective Action: The assessor that observed the violation and the agency Program Evaluator both gave technical assistance to correct the violation and prevent this from happening again. A Plan of Corrective Action was developed. The agency was put on notice that they cannot continue to be in violation regarding supervision without facing further consequences. Staff will be reminded of importance of supervision and the agency plans to continue the policy of educators sitting with the children while they eat, to maintain better focus on the children.

See Below for Detailed Information Observation: On November 8, 2017 a child was brought into the center with lipstick on her face and the parent said she could not get it off. A caregiver used a Mr. Clean Magic Eraser on the child's face causing a chemical burn or abrasion. No chemicals can be used on a child's skin and, in fact, anything put on a child's skin is to be by parental written permission only. While this was not done with the intention of hurting the child, the caregiver showed poor judgement and was immediately sent home. She has since been terminated from employment due to her actions. 1240-04-03-.07 STAFF Corrective Action: The caregiver was terminated from the program. The rest of the caregivers received more training on parental authorizations required for anything put on a child's skin, best practice, and licensing rules in general. Agency received technical assistance from PE and a written Plan of Corrective Action as well as a Put on Notice letter due to the incident.
