See Below for Detailed Information Observation: Children under the age of 4 years old were served whole baby carrots not cut up into bite sized pieces as required. 1240-04-01-.03 STAFF Corrective Action: The facility and all educators involved will be required to receive training by CCR&R. Observation: A child, under 2 years of age, was left unsupervised in the facility while the rest of the staff and children went outside to play. The Assessor was with the child during this time. 1240-04-01-.03 STAFF Corrective action: All educators involved will receive mandatory training from CCR&R.
