Observation: It was noted that there were seven children with one caregiver ages 6 months to 12 months in the infant B classroom. The ratio must be 1:4 ages 6 weeks to 15 months of age. The second caregiver had left the room without notifying anyone leaving the caregiver with too many children. This was corrected immediately before Program Evaluator left the classroom. 1240-04-03-.06 Supervision. Corrective Action: Agency will be training and reminding all staff that before they leave a classroom for any reason the caregiver must notify someone to replace them before they leave.

Observation: On March 21, 2018 during the assessment a caregiver in Infant A did not do a physical touch an infant who was asleep every 15 minutes. The caregiver missed the check by 5 minutes. Children in cribs must be physically touched every fifteen minutes by a caregiver. Observation: On March 29, 2018 it was noted by the Program Evaluator that in Infant B a 12 month old child who was in a crib a sleep with a blanket. No soft bedding such as blankets are permitted in cribs while infants sleep. 1240-04-03-.06 SUPERVISION Corrective Action: Staff will be trained by agency management on safe sleep procedures for Infant A and Infant B.
