See Below for Detailed Information Observation: Infant, aged eleven (11) months, enrolled and in care at the Agency. 1240-04-01-.06 STAFF. Plan of Corrective Action: Agency is not licensed for children under twelve (12) months of age. Management will directly oversee and approve enrollment of each child who enrolls at the Agency. This will include,verification of all documentation presented, Management will sign and date their approval when confirmation of records is achieved. Agency will only enroll children between the ages of twelve (12) months and eight (8) years. Observation: Infant, aged eleven (11) months, enrolled and in care at the Agency. 1240-04-01-.03 REQUIREMENTS FOR LICENSURE AS A CHILD CARE AGENCY. Plan of Corrective Action: Agency is not licensed for children under twelve (12) months of age. Agency will request that a copy of the child's birth certificate be presented for verification of the child's birth date prior to being enrolled. Agency will only enroll children between the ages of twelve (12) months and eight (8) years. Observation: Infant, aged eleven (11) months, was placed on a mat and not in a crib during naptime. 1240-04-01-.14 EQUIPMENT FOR CHILDREN. Plan of Corrective Action: Agency is not licensed for children under twelve (12) months of age. Cribs are not required for children twelve (12) months and older. Management will oversee enrollment for each child who is accepted into care at the Agency. Management will verify all information submitted and will sign and date showing acceptance. Observation: Infant, aged eleven (11) months, was placed on a mat and not in a crib during naptime. 1240-04-01-.11 SUPERVISION. Plan of Corrective Action: Agency is not licensed for children under twelve (12) months of age. The child reached the age of twelve (12) months prior to April 30, 2021. Safe Sleep Supervision will be followed for every child in Agency's care. The staff member who assists with the child's enrollment will submit the application information directly to Management for verification and approval. When all information is verified, Management will sign and date the approval for acceptance into Agency. Agency will only enroll children between the ages of twelve (12) months and eight (8) years.
