See Below for Detailed Information Observation: Food was not properly diced to prevent choking before serving to toddlers during breakfast as observed by DHS Assessor. Violation was corrected immediately upon intervention of Assessor and served properly during lunch service that same day. 1240-04-01-.13 Food and Food Service Corrective Action Plan: All agency staff will review CACFP choking prevention guidelines and Educators will review foods prior to allowing children access to them to ensure that no choking hazards are present. Observation: Management did not ensure the safety of children in care through proper evaluation and training of staff in compliance with licensure rules. 1240-04-01-.06 Staff Corrective Action: Management will schedule training with Child Care Resource and Referral and will regularly observe educators with children to properly evaluate their performance. Observation: Supervision lapse of 2 minutes and 26 seconds occurring in the toddler classroom when two children left the classroom without the educator's knowledge. 1240-04-01-.11 Supervision Corrective Action Plan: Educators will receive technical assistance in supervision from child care resource and referral specialist. Director will regularly evaluate staff to ensure that active supervision is occurring while educators interact with the children in their care.

See Below for Detailed Information Observation: The outdoor barrier wall on the playground was in need of repair with broken wood and protruding staples. 1240-04-01-.16 PHYSICAL FACILITIES Corrective Action: The agency will complete necessary repairs to the outdoor barrier wall by 10/8/18. They will discontinue the use of this area of the playground until the repairs are completed in full. Observation: The agency failed to file incident reports as required when a child at the agency received an injury requiring outside medical attention. 1240-04-01-.09 INCIDENT REPORTING. Plan of Correction: The agency will complete incident reports on this incident and file them as required. They will provide each classroom with blank incident reports to be completed and given to the parent/ guardian as required. Serious injury reports are now available in all classrooms.
