Unless appealed in writing to the Commissioner of the Department of Human Services, this Civil Penalty shall become final ten (10) calendar days after the date the order of assessment is served. Summary of Complaint Investigation Visit Today's visit included monitoring the agency's compliance with Licensing Rules and Regulations and monitoring a safety plan. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. The agency failed to report the concerns to the Department of Children's Services immediately as required by Tennessee Law and Licensing Rules & Regulations. On November 12, 2024 concerns of improper discipline techniques was reported at this agency with possible incidents occuring Nov. 8, 2024 The agency failed to report the incident(s) to the child's parent/guardian or authorized representative or to the appropriate regulatory agency immediately on the date of occurance.

See Below for Detailed Information Observation: A two (2) year old child walked out of the classroom into the hallway without supervision. 1240-04-01-.11 SUPERVISION. Corrective Action: Agency received training in supervision and will utilize this to ensure all children are properly supervised.
