Summary of Complaint Investagion and Safety plan Served Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and a complaint and serving safety plan. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans. During an unannounced visit on March 14, 2024, for the purpose of a complaint investigation, Child Care Licensing Consultant (CCLC Carmon) validated an agency self-reported incident by reviewing video footage from March 13, 2024, around 12:00PM. CCLC Carmon observed on the video footage that the educator did roll call outside the classroom after lunchtime. The educator opened the door and the children went inside the classroom. During an unannounced visit on March 14, 2024, for the purpose of a complaint investigation, Child Care Licensing Consultant (CCLC Carmon) validated an agency self-reported incident by reviewing video footage from March 13, 2024, around 12:00PM. See Below for Detailed Information Observation: Child, aged four (4) years, left unsupervised in the Dobbins E classroom for four (4) minutes and sixteen (16) seconds on January 26, 2022. 1240-04-01-.11 SUPERVISION. Corrective Action: Educators will complete one on one training with CCR&R on February 11, 2022 and will agree to use the techniques provided by CCR&R. Training Team Leader will review and retrain Educators on Safety and Supervision – Active Supervision Procedures (In-Service, February 28, 2022), UETHDA’s Standards of Conduct, Head Start Performance Standards (HSPS) 1302.90(c)(1)(v), and TDHS Licensing Regulations. Educators will agree to follow all Active Supervision Policies and Procedures. Education Manager had a conversation with Educator involved about developing additional measures to ensure compliance, which include (1) dots in hallways for children to line up for roll call and counting (2) checklist of children present for calling roll in addition to sign in/out log and (3) roll call will be conducted when transitioning out of the classroom. Observation: Child, aged four (4) years, left unsupervised in a for four (4) minutes and sixteen (16) seconds on January 26, 2022. 1240-04-01-.06 STAFF. Corrective Action: Educators will complete one on one training with CCR&R on February 11, 2022 and will agree to use the techniques provided by CCR&R. Training Team Leader will review and retrain Educators on Safety and Supervision – Active Supervision Procedures (In-Service, February 28, 2022), UETHDA’s Standards of Conduct, Head Start Performance Standards (HSPS) 1302.90(c)(1)(v), and TDHS Licensing Regulations. UETHDA is expanding their Active Supervision Policy to focus more closely on addressing “Transitions Out of the Classroom and Transitions Within the Classroom.” Educators will receive Active Supervision Training twice per year. Staff will be retrained on Supervision at UETHDA’s Annual Pre-Service Training – July 25 through July 29, 2022. Observation: Child, aged four (4) years, left unsupervised in a classroom for four (4) minutes and sixteen (16) seconds on January 26, 2022. 1240-04-01-.11 SUPERVISION. Corrective Action: Educators will complete Active Supervision Training twice per year, either in person or virtually with the use of Safe Schools. Educators will be retrained on Active Supervision Policies and Procedures at the UETHDA In-Service on February 28, 2022. Educators will sign an agreement stating that they agree to follow all Active Supervision Policies & Procedures. Each year UETHDA’s Standards of Conduct is reviewed with all Head Start staff and they sign that “I have read and understand the contents in the Standard of Conduct Policy of UETHDA. I agree to comply with all provisions of the Standard of Conduct during my Contract with UETHDA.” The Standard of Conduct Policy states that “Leaving a child alone or unsupervised while under the program’s care,” is considered an unacceptable activity.

See Below for Detailed Information Observation: A child was left unsupervised in the classroom for approximately 23 seconds when educators took the remaining children out of the classroom without performing a proper roll call before leaving the room. 1240-04-01-.11 SUPERVISION. Corrective Action Plan: Management will reinforce supervision policy and procedures with staff including additional training and Corrective Action Plan for educators who were responsible for the lapse of supervision. TECTA is providing coaching to staff on supervision procedures.

See Below for Detailed Information Observation: The recording of the time that the children were released from the bus was not documented on transportation log on this date, as well as multiple days from the previous months. 1240-04-01-.17 TRANSPORTATION. Corrective Action: Agency will retrain their Educators and Staff, with the assistance of TECTA, on proper transportation procedures. Observation: A child left adult supervision for approximately one minute without the supervising educator's knowledge. 1240-04-01-.11 SUPERVISION. Corrective Action: Educators involved in the Lapse of Supervision have been terminated from the Agency. The agency staff will be retrained. Observation: Multiple children had not been signed into the agency by staff upon their arrival to the agency by bus. 1240-04-01-.11 SUPERVISION. Corrective Action: Agency will retrain their Educators and Staff, with the assistance of TECTA, on the proper Sign In / Out Procedures. Observation: Agency Management failed to ensure compliance with signing children into and out of the program as well as the completion of daily transportation logs. 1240-04-01-.06 STAFF. Corrective Action: Agency will retrain Educators and Staff, with the assistance of TECTA, on the proper Sign In / Out Procedures (Supervision) and Unloading Procedures (Transportation).

See Below for Detailed Information Observation: A child was left unsupervised for approximately 40 seconds when educators left the classroom with the rest of the group without conducting a roll call to ensure that all children were present. 1240-04-01-.11 SUPERVISION Corrective Action Plan: All staff will be retrained in supervision procedures and requirements to ensure child safety. Observation: The agency failed to provide documentation of an incident involving a supervision lapse to the parent/guardian of the child by the release of the child the same day as required. 1240-04-01-.09 INCIDENT REPORTING. Corrective Action Plan: The agency will retrain all staff on the requirements for incident reporting. Observation: Management failed to ensure that all staff were consistently following requirements for supervision procedures and incident reporting. 1240-04-01-.06 STAFF Corrective Action Plan: DHS Licensing staff met with the management to clarify licensing regulations in Incident Reporting and Supervision. Training in supervision procedures and incident reporting will be provided to all staff.
