See Below for Detailed Information Observation: Management of the agency failed to appropriately plan staff schedules to ensure supervision of the children at all times. 1240-04-01-.06 Staff Corrective Action Plan: The director of the agency has adjusted staff schedules so that there will be a sufficient number of educators present to meet supervision requirements at all times. Observation: An educator did not provide direct focus to children who were eating as required to reduce the risk of choking and be able to respond immediately if necessary. 1240-04-01-.11 Supervision Corrective Action Plan: The agency's director has adjusted staff schedules to ensure that enough educators are present to maintain required supervision levels at all times. Observation: On September 13, 2019, the agency did not provide an additional educator to supervise children and instead relied upon on the cook who is not permitted to be counted in adult:child ratio requirements while preparing the meal. 1240-04-01-.11 SPECIFIC REQUIREMENTS FOR CHILD CARE CENTERS. Corrective Action Plan: The agency's director has adjusted staff schedules to ensure that enough staff are present at all times to maintain required supervision levels of the children.

See Below for Detailed Information Observation: On February 12, 2019, the Department received notification from DHS Assessment that four incidents of mealtime supervision lapses were observed in the infant/toddler classroom as the educators were not providing direct focus to children during the meal as required for their safety. 1240-04-01-.11 SUPERVISION Corrective Action Plan: Management has retrained educators in mealtime supervision procedures and is reviewing staffing patterns to provide acceptable supervision at all times. Observation: Management of the agency did not ensure that proper mealtime supervision procedures are always followed to ensure child safety. Direct focus and supervision to ensure child safety was not provided at all times to infants and toddlers who were eating breakfast during the agency's assessment on 2/12/19. 1240-04-01-.06 STAFF Corrective Action Plan: Management has retrained educators in mealtime supervision procedures and is reviewing staffing patterns to provide acceptable supervision at all times.
