Summary of Complaint Investigation with DCS / SIU and serving of two Safety Plans. The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and a complaint along with serving two individual safety plans. The agency understands that the safety plans are in place until a lift letter is received from DHS Licensing. Check Sheet reviewed with Director. Video footage was reviewed during a complaint investigation. Video footage revealed an educator mishandling a child while attempting to redirect the child.

See Below for Detailed Information Observation: Agency did not properly sanitized bottles before and after each use. 1240-04-01-.13 FOOD AND FOOD SERVICE. Plan of Corrective Action: Agency will retrain staff to follow 3 step sanitization process. Agency will also ask parents to bring prepared bottles to use. Observation: Child was drinking a bottle without proper mealtime supervision and an adult holding him. 1240-04-01-.13 FOOD AND FOOD SERVICE. Plan of Corrective Action: Agency staff will attend CCR&R training on mealtime supervision and follow meal time supervision plans. Observation: PE observed an infant sleeping in a bouncy seat instead of in approved napping equipment. 1240-04-01-.11 SUPERVISION. Plan of Corrective Action: Agency staff will attend CCR&R training on SIDS and Safe Sleep. Observation: Infant was on a boppy pillow drinking a bottle without an adult holding him. 1240-04-01-.13 FOOD AND FOOD SERVICE. Plan of Corrective Action: Staff will receive training in meal time procedures and supervision to ensure children are supervised to prevent choking. Observation: Staff did not follow meal time supervision plan as an infant was drinking a bottle without supervision on a boppy pillow. 1240-04-01-.06 STAFF. Plan of Corrective Action: Agency staff will attend CCR&R training on mealtime supervision and follow meal time supervision plans.

Summary of Complaint PCS1003002 and Serving of Safety Plan The visit conducted today included monitoring of the Agency's compliance with Licensing Rules and Regulations. Observation: On May 3, 2023, CACL received a complaint, and an investigation has been initiated involving one (1) Educator. Accordingly, Safety Plans have been implemented during the investigative period. Safety Plans, initiated on May 3, 2023, will remain in effect until TDHS complaint investigation is completed, and the outcome is determined. Simmons Ridge Children's Academy understands that it must abide by the terms of the Safety Plans until it is lifted in writing by TDHS. Simmons Ridge Children's Academy understands that the failure to abide by the terms of the Safety Plans will result in legal enforcement action against the Agency license, including the summary suspension of the license to operate. Technical Assistance provided- Licensing Consultant discussed Safety Plan and using the supervision log. Licensing Consultant gave contact information for the Child Care Resource & Referral CCR&R family engagement/involvement coach.
