Summary of Complaint Visit The visit conducted today included monitoring of the agency's compliance with Licensing Rules and Regulations and complaint investigation. If any violations were received, details can be observed in the compliance details section of the visit along with corresponding corrective action plans The child care agency failed to report to DCS within the required timeframe and following required procedure. The agency did not fully cooperate with all agencies involved in the investigation of child abuse or neglect including preventing appropriate investigators from the Department of Human Services and the Department of Children's Services from conducting interviews with children and staff at the agency. Agency management failed to ensure that staff consistently provided appropriate supervision to children in their care resulting in a lapse in required oversight. Agency failed to report a serious supervision incident to parents upon pick up and failed to report to DHS as required. Evidence gathered including video surveillance and verbal admissions from multiple agency staff during LC Shrum's investigation has confirmed that a serious incident occurred on March 4th, 2026 involving a 2 1/2-year-old child walking out of the agency without the knowledge of the staff and was left unattended outside of the building for at least 15 minutes.

Observation: Staff failed to report reasonable suspicions of physical abuse on a child to the Department of Children Services and/or local law enforcement. 1240-04-01-.10 Duty to Report Child Abuse and Neglect Corrective Action: The agency will conduct training with all staff on the proper procedures to follow for mandated reporting of child abuse and neglect of a child.

See Below for Detailed Information Observation: The information in three children's records regarding birth dates was not accurate. 1240-04-01-.08 RECORD KEEPING Corrective Action: The agency will have parents update the children's file with correct and current information, including the birth dates. Observation: Eight (8) children were present in the 2-4 year old room, one (1) of the children was actually 22 months. Toddlers are not to be included in the grouping of 2-4 year old. In the 3-5 year old class, two (2) of the children were younger than three (3) years and twelve (12) children enrolled. When children younger than 36 months are included the maximum number allowed is eleven (11) children. 1240-04-01-.22 SPECIFIC REQUIREMENT FOR CHILD CARE CENTERS Corrective Action: The agency will review the ages of the children enrolled and revise the grouping to meet the required ratio and group size charts.

See Below for Detailed Information Observation: School age children were included in the the preschool classroom, fifteen (15) children were present. When school age children are included the grouping, the maximum number allowed is ten (10) children. 1240-04-03-.06 SUPERVISION Corrective Action: Owner decided to eliminate her school age program, keeping only preschool children. This will also keep her number in attendance with in her license capacity. Observation: All children present must be counted in the licensed capacity. The total number of children in attendance was twenty-five (25), the licensed capacity is twenty-four (24). No more than twenty-four (24) children can be present at any time. 1240-04-03-.06 SUPERVISION Corrective Action: Owner decided to eliminate her school age program, keeping only preschool children. This will also keep her number in attendance within her license capacity. Observation: In addition to time out and redirection for behavior management, staff may hit a wooden ruler against a hard surface to gain the children's attention and threaten to send them to an imaginary "principal's" office. This is considered threatening and not appropriate behavior management. 1240-04-03-.09 PROGRAM Corrective Action: Agency will use time out and redirection for behavior management along with praise and encouragement for good behavior. Training in classroom management and age appropriate discipline will be required of all staff. Observation: Children are moved from one classroom to another classroom each day to accommodate the school age children that arrive each afternoon. Children should be assigned and remain with the same group throughout the day. 1240-04-03-.06 SUPERVISION Corrective Action: Owner decided to eliminate her school age program. The children will be assigned and remain in the same classroom during the day.
